運動傷害使關節炎年輕化！ 及早治療搭PLT治療早日康復
關節炎的困擾不僅限於老年人，年輕族群由於長時間的運動傷害、過度重訓或長期姿勢不當等，也可能成為關節炎的受害者。光仁骨外科診所院長陳國光指出，隨著現代生活方式的改變，退化性關節炎在年輕人中的發生率逐漸攀升，特別是長時間進行粗重工作或是長期大量運動和重訓的人，掌握正確的認知並及早治療才能避免關節炎影響生活品質。 ▲陳國光醫師指出，隨著現代生活方式的改變，關節炎在年輕人中的發生率逐漸攀升。（攝影／林志遠） 【NOW健康 陳木森／台中報導】 關節炎以膝關節疼痛居多 關節損傷分5級 膝關節是一個複雜的結構，由股骨、脛骨、髕骨、前後十字韌帶以及半月狀軟骨所構成，十字韌帶主要負責穩定和支撐，軟骨則覆蓋在骨頭的末端，有減輕磨擦和吸收衝擊的作用，內外側韌帶則有助於維持穩定性。 陳國光醫師指出，關節炎可分為創傷性、退化性、類風濕性、痛風性關節炎，常見的關節炎症狀是以膝關節疼痛居多。大部分的關節炎是由半月狀軟骨開始退化，進而影響其他部分，最終可能導致骨刺形成，進而影響關節的穩定性，值得注意的是，現代有年輕人患上創傷性關節炎的情況也日益增加。若以關節損傷程度來看，又可分為5級： ▸第1級：這個階段可能出現疼痛，但X光檢查可能無法顯示出明顯的變化。 ▸第2級：需要進行核磁共振等檢查，以觀察關節是否有變化。 ▸第3級：X光可能顯示出明顯的組織變化，同時可能出現軟骨磨損的跡象。 ▸第4級：骨刺可能變得更嚴重，關節面可能變窄，且腳有輕微變形等問題。 ▸第5級：嚴重疼痛、關節面狹窄，甚至造成活動受限。 關節損傷3級以上 應積極介入治療勿拖延 對於不同程度的關節炎，治療方式也有所不同，陳國光醫師提到，輕度的1、2級關節炎可透過藥物治療，但隨著病情的加重；到了3級以上則需要更積極的物理治療、復健，甚至是體外震波、玻尿酸、PRP、PLT等治療；面對不想動手術或因為體況不適合動手術的4、5級關節炎病人，此時物理治療、體外震波和玻尿酸的幫助較不明顯，建議積極採用PRP或PLT進行治療；愈早接受醫療介入愈能夠感受到效果，拖愈久反而療程效力會降低。 在眾多治療方式之中，PLT高濃度血小板凍晶治療逐漸成為受歡迎的選擇。陳國光醫師解釋，PLT是一種由患者自身的血液中提取製成的血小板凍晶，血小板中含有豐富的生長因子，具有幫助組織再生的作用，相較於傳統的PRP，PLT的優勢在於每瓶皆擁有高達10億個血小板，經過標準化的萃取，品質與劑量皆更為穩定。醫師強調，PLT治療是一個簡便且相對無痛的療程。患者只需抽血1次，就可以製作10到20瓶凍晶，足夠進行多次治療，病患應於療程後勤勞冰敷患處，以減少腫脹，治療後的第1天，病患需避免泡澡或劇烈運動，後續則可逐漸恢復正常生活。 ▲陳國光醫師特別強調，PLT治療的最佳時機是在關節炎初期，因此無論年紀大或小，只要意識到關節有任何不適，就應盡速就醫尋求專業檢查及治療。（攝影／林志遠） 關節炎初期搭PLT治療 患者感受改善顯著 根據陳國光醫師的經驗，PLT治療在膝關節治療的應用中表現優異，許多患者在治療後有明顯的改善。病例中有的患者原本需要拄拐杖，治療後不僅能夠正常行走，而且X光檢查也顯示軟骨狀態明顯改善。 陳國光醫師特別強調，PLT治療的最佳時機是在關節炎初期，因此無論年紀大或小，只要意識到關節有任何不適，就應盡速就醫尋求專業檢查及治療，對抗關節炎需要專業醫師綜合評估與後續的定期治療，以找回關節的健康狀態。
